Partnership in Health Agreement and Waiver
I, , hereby release True Life Quest LLC (The Company) and Daniel Martinez Stahl (The Facilitator) from any liability or claims that could be made against the company or him concerning my mental and/or physical well-being during the work that has been outlined and agreed upon (now and in the future) by filling out this form.
Partnership in Health: I understand that participation in any session or program with Daniel Martinez Stahl indicates that I am willing and ready to look within myself for the answers, that his role is to help me understand the elements that make my experience of life possible and of how the mind works, so that I may find and connect more deeply with my own true self, my inner wisdom, and my innate well-being. I understand that deepening my spiritual intelligence includes an understanding of my spiritual self in this current life, as well as the exploration of my spiritual journey, which can be remembered via regression to my past lives and life between lives. I understand that whatever I am able to experience through spiritual regression is directed and made possible by my own higher self along with my spiritual guides, exposing me to the most helpful information that is relevant and appropriate for my current situation and spiritual development.
Participation: I give Daniel Martinez Stahl full permission to coach me and offer channeled messages, guided visualizations, meditations, and relaxation techniques, knowing that by engaging and participating fully in the process, I play an important role in my overall success.
Legal Release: I am of legal age and in consideration of my own acceptance as a participant in hypnosis, hypnotherapy, guided meditation, regression sessions, channelings, energy therapies, workshops, seminars, or related activities. I for myself, my heirs, executors, administrators, and assignees, do hereby release and discharge True Life Quest LLC and Daniel Martinez Stahl from all claims of liability, damages, demands, and actions whatsoever in any manner arising from or growing out of my participation. I also understand that any advice or counseling given, while within the parameters of current coaching, guidance and modern hypnotherapeutic practice, is accepted and/or acted upon entirely at my own risk. In addition, I understand that, while safe, highly successful, proven, cutting edge techniques are utilized, no guarantee of any kind whatsoever is implied, promised, or bestowed.
Guarantee: I understand that True Life Quest LLC and Daniel Martinez Stahl cannot and does not guarantee results since my own personal success depends on many factors that Daniel Martinez Stahl has no control over, including my willingness and desire to affect the changes inside of myself.
Medical Release: I confirm that I have no medical or psychiatric condition which could prevent me from safely experiencing hypnosis, guided meditation, regression, and/or related activities, and I further understand that such a session in no way supersedes any medical or other treatment I may be undergoing from a medical or health practitioner. I also agree that any hypnosis, guided meditation, regression, and/or related session is purely voluntary on my part.
Scope of Practice: I understand that Daniel Martinez Stahl is not a licensed physician, psychologist, or medical practitioner of any kind and that coaching, channeling and hypnosis should not be considered a replacement for the advice and/or services, of a psychiatrist, psychologist, psychotherapist, or doctor.
Digital Recording(s): I give True Life Quest LLC and Daniel Martinez Stahl full permission to make an electronic record of me as a normal part of participation in hypnosis, hypnotherapy, guided meditation, regression sessions, energy therapies, seminars, or related activities. I agree that this electronic record is mutually owned by me and True Life Quest LLC, and may not be copied, published, distributed, or disseminated in any manner, without joint and mutual express permission. I understand that this record, if held by True Life Quest LLC and Daniel Martinez Stahl, will be kept in strictest confidence, privacy, and secrecy, and will not be given, shown, or shared with any person or organization, except office staff, law enforcement, legal counsel, or in other legal processes, without my express permission.
Should I agree for my audio/video to be shared with others for educational purposes, I hereby grant permission to True Life Quest LLC and Daniel Martinez Stahl the rights of my image, in video or still and of the likeness and sound of my voice as recorded on audio or video without payment or any other consideration. I also understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.
Deepening Process (during in-person sessions): I hereby grant permission to Daniel Martinez Stahl to respectfully lift my arms and touch my shoulders and head in order to help facilitate the deepening process.
Confidentiality: By signing this form, I consent that True Life Quest LLC and Daniel Martinez Stahl may release information to a specific individual or agency if it has been determined that a child or elder is at risk of or is currently being abused; if I, as a client, am in imminent danger to myself or others; or if a subpoena of records has been requested.
Information Release: The information that I recall during a session may be used by True Life Quest LLC and Daniel Martinez Stahl for research, writing, promotion and/or speaking engagements to inform, educate and enlighten others as long as my name, address and identifying details are never used. The audio recordings may be shared with a mentor or teacher for the sole purpose of assisting Daniel Martinez Stahl with his own development and improvement as a facilitator and practitioner. I also understand that, at any time, Daniel Martinez Stahl may discuss aspects of my case with other colleagues, always keeping my full name and identity completely confidential unless I have given permission otherwise.
Refund Policy: In the event of a participant deciding not to continue with the course or program, the participant will be entitled to a refund of 100% of that part of the training or sessions paid for but not received, excluding the initial non-refundable deposit (US$222). Any training or sessions already received but not paid for, will become payable. If the course or program payments are in arrears at the time of completion, any relevant certification may be delayed at our discretion.

Agreement to all the stipulations listed in this document is given voluntarily and constitutes an irrevocable waiver of any privilege otherwise attached to the subject matter of this document, extending itself to my heirs and personal representatives. My signature at the bottom of this form attests to my unconditional acceptance of and agreement to all of the above sections, unless otherwise noted.
